
Getting started with SEBB Premera insurance enrollment is easier than you think. To be eligible, you must be a State of Washington employee or a retiree of the state.
First, you'll need to create an account on the Premera website. This will give you access to your personal plan information and allow you to enroll or make changes to your coverage.
Enrollment and Sign-up
To enroll in SEBB Premera, you'll need to submit forms A & B to The Washington Health Care Authority. You can do this by mail, fax, or electronic submission.
To mail your forms, send them to Washington State Health Care Authority, PO Box 42684, Olympia, WA 98504-2684. Alternatively, you can fax them to 360-725-0771. If you prefer to submit your forms electronically, you can send a secure online message to PEBB Customer Service by registering for an account at www.hca.wa.gov/fuze-questions.
If you're having trouble with your account, you can also sign in or set one up by mail or fax. To do this, send your request to Premera Blue Cross at PO Box 91120, MS295, Seattle, WA 98111-9220, or fax it to 425-918-5278.
If you're new to the process, you can download enrollment forms from the Health Care Authority website. If you need help, you can also call Premera Blue Cross at 888-208-6264 (TRS: 711) to receive an enrollment kit by mail.
Blue Cross and MultiCare Contract Update

Premera Blue Cross and MultiCare have reached an agreement, ensuring no break in coverage for Premera members enrolled through the School Employees Benefits Board (SEBB) Program.
This agreement affects Premera members enrolled through the SEBB Program, as well as Uniform Medical Plan (UMP) members in Spokane County enrolled through the Public Employees Benefits Board (PEBB) and SEBB Programs.
Premera Blue Cross announced this agreement on May 24, 2024.
How to Enroll
To enroll in a plan, you can submit your forms A & B to The Washington Health Care Authority. There are three ways to do this: by mail, fax, or electronic submission.
You can send your forms by mail to Washington State Health Care Authority, PO Box 42684, Olympia, WA 98504-2684. If you prefer to fax, the number is 360-725-0771.
For electronic submission, you'll need to send a secure online message to PEBB Customer Service by registering for an account at www.hca.wa.gov/fuze-questions. Be sure to date any forms attached to your message.

If you need to sign in or set up an account, you can do so by contacting Premera Blue Cross. They can be reached by mail at PO Box 91120, MS295, Seattle, WA 98111-9220, or by fax at 425-918-5278.
